Support for Living Well at Home
Staying in your own home is important, and Ginger Cove at Home helps ensure your home continues to meet your needs. As a member, you receive an occupational therapy-led home safety evaluation, along with recommendations for modifications to support safe, confident living at home.
Care Coordination
Care coordination services are designed to support you over time – helping identify potential healthcare needs early and working alongside you and your physician to create a thoughtful care plan that evolves as your needs change.
Member Choice for Healthcare Providers
When you need services, such as home health care or rehabilitation in a skilled nursing facility, you are free to choose your preferred provider. Your Ginger Cove at Home plan will reimburse you for the services.*
*Reimbursement is plan dependent
